ABSTRACT

Objective To investigate the current status of hepatitis B virus (HBV) infection among hospitalized patients aged 1-18 years, as well as the status of immunity after hepatitis B vaccination. Methods Related data were collected from the patients aged 1-18 years who were hospitalized in Henan Provincial People's Hospital from July 2020 to July 2021, including serological markers for hepatitis B (HBsAg, anti-HBs, HBeAg, anti-HBe, and anti-HBc) and hepatitis B vaccination. The epidemiological situation of HBV infection was analyzed, as well as the immune effect after vaccination. The trend chi-square test was used for trend analysis. Results A total of 10 658 hospitalized patients were collected, among whom there were 6 372 male patients (59.79%) and 4 286 female patients (40.21%). In this population, the patients with positive HBsAg accounted for 0.28% (30/10 658), with a relatively high proportion of 0.68% and 0.62%, respectively, in the 17-and 18-year age groups; the patients with positive anti-HBs accounted for 51.82% (5 523/10 658), with a relatively high proportion of > 63% in the 1-4 years age groups, and there was a reduction in the proportion of patients with positive anti-HBs (fluctuating around 40%) in the 5-18 years age groups. With the increase in age, the positive rate of anti-HBs tended to decrease in both male and female patients (male: χ 2 =8.217, P =0.004; female: χ 2 =10.048, P =0.002). Conclusion Based on the data of hospitalized patients, HBV infection in the population aged 1-18 years in Henan Province has the characteristics of low prevalence rate and high immunity, and the reduction in the proportion of patients with positive anti-HBs at more than five years after vaccination should be taken seriously in this region.

ABSTRACT

Objective: To evaluate the efficacy of supraclavicular fasciocutaneous island flap (SIF) for repairing the defect of parotid or auricle regions after tumor resection. Methods: From February 2019 to June 2021, 12 patients (11 males and 1 female, aged 54-77 years old), of whom 4 with parotid adenoid cystic carcinoma and 8 with auricular basal cell carcinoma underwent reconstruction surgery for postoperative defects in the parotid gland area and auricular area with SIF in the Department of Otorhinolaryngology Head and Neck Surgery, the Second Xiangya Hospital of Central South University and their clinical data were retrospectively analyzed. Size of the SIF, time for harvesting SIF, neck lymph node dissection and postoperative complications were recorded. Results: The flap areas were (6-9) cm × (8-13) cm, and the harvesting time for SIF ranged from 40 to 80 min, averaging 51.7 min. The donor sites were directly closed. All patients underwent ipsilateral levels Ⅰ-Ⅲ neck dissection, with 4 cases undergoing additional level Ⅳ neck dissection and 2 cases undergoing level Ⅳ-Ⅴ neck dissection. Of the 12 SIF, 10 were completely survival and 2 had flap arterial crisis with partial flap necrosis, in addition, 1 had donor site wound dehiscence. With follow-up of 10-42 months, there were no tumor recurrences in 10 patients, 1 patient was lost to follow-up at 10 months postoperatively, and 1 patient experienced local tumor recurrence at 11 months after surgery and died 15 months later. Conclusion: SIF is an easily harvested flap with good skin features matching the skin in parotid and auricle regions and less damage to donor site, and this flap has no need for microvascular anastomosis technique. SIF is feasible and effective for repairing defects in parotid and auricle area.

ABSTRACT

OBJECTIVE@#To explore the clinical characteristics and genetic basis for three children with Congenital chlorine diarrhea (CCD).@*METHODS@#Three children with CCD who attended the Affiliated Children's Hospital of Capital Pediatric Institute from June 2014 to August 2020 were selected as the research subjects. Peripheral blood samples of the three children and their parents were collected for genetic testing. And the results were verified by Sanger sequencing.@*RESULTS@#The clinical manifestations of the three children have included recurrent diarrhea, with various degrees of hypochloremia, hypokalemia and refractory metabolic alkalosis. Genetic testing revealed that the three children have all carried variants of the SLC26A3 gene, including homozygous c.1631T>A (p.I544N) variants, c.2063_1G>T and c.1039G>A (p.A347T) compound heterozygous variants, and c.270_271insAA(p.G91kfs*3) and c.2063_1G>T compound heterozygous variants. Sanger sequencing confirmed that all of the variants were inherited from their parents.@*CONCLUSION@#The variants of the SLC26A3 gene probably underlay the CCD in these children. Above finding has enriched the spectrum of SLC26A3 gene variants.

ABSTRACT

Objective: To evaluate the efficacy and safety of hybutimibe monotherapy or in combination with atorvastatin in the treatment of primary hypercholesterolemia. Methods: This was a multicenter, randomized, double-blind, double-dummy, parallel-controlled phase Ⅲ clinical trial of patients with untreated primary hypercholesterolemia from 41 centers in China between August 2015 and April 2019. Patients were randomly assigned, at a ratio of 1∶1∶1∶1∶1∶1, to the atorvastatin 10 mg group (group A), hybutimibe 20 mg group (group B), hybutimibe 20 mg plus atorvastatin 10 mg group (group C), hybutimibe 10 mg group (group D), hybutimibe 10 mg plus atorvastatin 10 mg group (group E), and placebo group (group F). After a dietary run-in period for at least 4 weeks, all patients were administered orally once a day according to their groups. The treatment period was 12 weeks after the first dose of the study drug, and efficacy and safety were evaluated at weeks 2, 4, 8, and 12. After the treatment period, patients voluntarily entered the long-term safety evaluation period and continued the assigned treatment (those in group F were randomly assigned to group B or D), with 40 weeks' observation. The primary endpoint was the percent change in low density lipoprotein cholesterol (LDL-C) from baseline at week 12. Secondary endpoints included the percent changes in high density lipoprotein cholesterol (HDL-C), triglyceride (TG), apolipoprotein B (Apo B) at week 12 and changes of the four above-mentioned lipid indicators at weeks 18, 24, 38, and 52. Safety was evaluated during the whole treatment period. Results: Totally, 727 patients were included in the treatment period with a mean age of (55.0±9.3) years old, including 253 males. No statistical differences were observed among the groups in demographics, comorbidities, and baseline blood lipid levels. At week 12, the percent changes in LDL-C were significantly different among groups A to F (all P<0.01). Compared to atorvastatin alone, hybutimibe combined with atorvastatin could further improve LDL-C, TG, and Apo B (all P<0.05). Furthermore, there was no significant difference in percent changes in LDL-C at week 12 between group C and group E (P=0.991 7). During the long-term evaluation period, there were intergroup statistical differences in changes of LDL-C, TG and Apo B at 18, 24, 38, and 52 weeks from baseline among the statins group (group A), hybutimibe group (groups B, D, and F), and combination group (groups C and E) (all P<0.01), with the best effect observed in the combination group. The incidence of adverse events was 64.2% in the statins group, 61.7% in the hybutimibe group, and 71.0% in the combination group during the long-term evaluation period. No treatment-related serious adverse events or adverse events leading to death occurred during the 52-week study period. Conclusions: Hybutimibe combined with atorvastatin showed confirmatory efficacy in patients with untreated primary hypercholesterolemia, which could further enhance the efficacy on the basis of atorvastatin monotherapy, with a good overall safety profile.

ABSTRACT

The paper introduces professor GAO Shu-zhong's understanding on "seeking yin from yang needling method" and its clinical application on the basis of "qi street" and "four seas" theories. Through professor GAO's clinical practice for years, he integrates and extendes the theories of "seeking yin from yang", "qi street" and "four seas" in Huangdi Neijing (The Yellow Emperor's Inner Classic). In this specific acupuncture method, in reference with the theories of "qi street" and "four seas", acupuncture is exerted on yang part of body, e.g. the back and lumber region to treat the diseases of yin parts, e.g. the chest and abdomen, which is differentiated as yin-yang imbalance in pathogenesis. In order to fully explain the clinical curative effect of "seeking yin from yang needling method", the common diseases in clinic, e.g. the disorders of heart, spleen and stomach systems, as well as the gynecology are taken as examples in the paper.

ABSTRACT

This study is to establish and validation in vivo models of moxifloxacin based on the theory of physiologically based pharmacokinetics (PBPK), and then to predict the distribution of moxifloxacin in human venous return and organ such as lung, spleen and so on. The efficacy of moxifloxacin and its pharmaceutical preparations were quantified by comparing the pharmacokinetic parameters with the minimum inhibitory concentration of related pathogenic bacterium. The results showed that the anti-infection efficacy of pharmaceutical moxifloxacin preparation in the corresponding organs was basically the same. The PBPK model of moxifloxacin preparations can be more accurately described the pharmacokinetic of anti-infective drugs in human, it is suitable for the efficacy evaluation of anti-infective drugs and provides a strong basis for the corresponding scientific research and scientific supervision.

ABSTRACT

A 6-month-oldgirl was admitted in Affiliated Children′s Hospital of Capital Institute of Pediatrics with the complaint of “Recurrent fractures within 3 months”. She presented with frequent fractures, skeletal deformities,and distinctive facial features, including wide forehead, ocular proptosis and a flat nose bridge. She was diagnosed as osteoporosis imperfecta based on the clinical characteristics and given pamidronate disodium treatment. The whole exon sequencing showed heterozygous mutation of P4HB gene c.1178A>G (p.Y393C), which leads to a rare type of osteoporosis imperfect a Cole-Carpenter syndrome-1. Eight cases of osteoporosis imperfecta affected by P4HB mutation involving 5 mutationsites were retrieved from literature review. Different mutation sites lead to different clinical manifestations and severity of disease. The genotype-phenotype correlation of osteoporosis imperfect may be associated with the domains of coding proteins.

ABSTRACT

Objective:To explore the relationship between uric acid (UA), transforming growth factor-β (TGF-β), nerve growth factor (NGF) and bipolar disorder (BPD) and its clinical significance.Methods:From January 2019 to January 2020, 100 BPD patients in the First Hospital of Hebei Medical University were selected as the observation group, and 100 healthy subjects were selected as the control group. The general data, serum UA, TGF-β, and NGF levels were compared between the two groups, and the factors affecting the incidence of BPD and the correlation between serum UA, TGF-β, and NGF were analyzed. And the diagnostic value of serum UA, TGF-β, and NGF on BPD and BPD patients with mixed episodes of depression and mania was analyzed.Results:There were statistically significant differences in the levels of fasting blood glucose, triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), UA, TGF-β and NGF between the two groups ( P<0.05). Logistic regression analysis showed that fasting blood glucose, TG, TC, HDL-C, LDL-C, UA, TGF-β, and NGF levels were all influencing factors of BPD ( P<0.05). Serum UA of BPD patients was negatively correlated with TGF-β and NGF ( r=-0.744, -0.754, P<0.05), and TGF-β was positively correlated with NGF ( r=0.824, P<0.05). The area under the curve (AUC) values of serum UA, TGF-β, and NGF in the diagnosis of BPD were all high, especially the highest in combined diagnosis, reaching 0.844. The serum UA level of the mixed episode group was higher than that of the single episode group, and the serum TGF-β and NGF levels were lower than that of the single episode group, the difference was statistically significant ( P<0.05). Serum UA, TGF-β, and NGF levels had high diagnostic value for depression and manic mixed episodes in BPD patients, and the combined diagnosis had the highest AUC value, reaching 0.770. Conclusions:The serum UA, TGF-β, and NGF of BPD patients are all abnormally expressed, which may be biomarkers of BPD, and there is a certain correlation between the indicators. The combined examination can improve the diagnostic value of mixed episodes of depression and mania in BPD and BPD patients, and provide a basis for early diagnosis and treatment.

ABSTRACT

Research on polymer impurities has always been important in the quality control of cephalosporins. Research on polymers in cephalosporins that lack active amino groups on the C-7 side chain has not been reported. Therefore, our study used cefazolin sodium, which is widely used in the clinic, as an example. The polymer in cefazolin sodium and its product "cefazolin sodium pentahydrate for injection" was analyzed by column switching liquid chromatography-high resolution mass spectrometry. Two polymer impurity peaks were detected and the possible structures of these polymers were suggested. Through two-dimensional liquid chromatography, the chromatographic peaks following Sephadex gel chromatography and high-performance gel chromatography were compared to those obtained by reverse high-performance liquid chromatography (HPLC) for cefazolin sodium as reported in the Chinese Pharmacopoeia. The HPLC method proves more suitable for polymer detection than Sephadex gel chromatography and high-performance gel chromatography. The method of polymer detection for cefazolin sodium was established using the method of related substances HPLC as described in the Chinese Pharmacopoeia.

Aims@#This study aimed to isolate and characterize putative new probiotic with antimicrobial properties against common fish pathogens from the gut of Oreochromis spp. (red tilapia). @*Methodology and results@#A total of 28 colonies were isolated from gut of Oreochromis spp. and characterized phenotypically. Eight isolates were selected for probiotic characterization. Temperature, salinity, pH and bile salt tolerance, antibiotic susceptibility and antimicrobial test against selected fish pathogens (Aeromonas hydrophila, Edwardsiella tarda and Staphylococcus aureus subsp. aureus ATCC 25923) were conducted. Characterization studies revealed isolates suited for freshwater environment and exhibited tolerance against wide range of salinity, pH and bile salt. Isolates displayed different antibiotic susceptibility profile, with six exhibited antimicrobial properties against E. tarda. Molecular identification based on 16S rRNA gene sequencing showed 99.44%, 98.59% and 91.21% sequence similarity with Leuconostoc pseudomesenteroides strain 3832T, Leuconostoc lactis strain KCC202369T and Leuconostoc mesenteroides strain 4332T, respectively as compared to known sequence in the GenBank. When identified Leuconostoc spp. were coated on feed pellets, no major decrease in viability over 21 days of storage at 4 °C were observed, with an average of 8 log CFU/mL.@*Conclusion, significance and impact of study@#The characterized species allow further application assessment of the probiotic-supplemented tilapia feed. Host-originated Leuconostoc displayed potential antimicrobial properties against fish pathogen E. tarda. The isolates Leuconostoc is expected to provide protective effect for Oreochromis spp. against edwardsiellosis and to exert beneficial effects more efficiently as compared to commercial probiotics which are not specifically target for Oreochromis spp., thereby indirectly helping fish farmers in achieving economic sustainability and increase affordability of fish.

ABSTRACT

Objective: Mulberry (Morus spp.) fruits and leaves have been proven to possess nutraceutical properties. Due to its fast and easy growing characteristics, mulberry fruits (MF) and leaves (ML) potentially emerge as a great source of functional foods. This study aims to enhance bioactivities (antioxidant, anti-inflammation, and hypoglycemic activity) of MF and ML via submerged fermentation using bacteria (Lactobacillus plantarum TAR 4), yeast (Baker's yeast and red yeast) and fungi (Tempeh and Tapai starter). Methods: In this study, 25% (mass to volume ratio) of MF and ML were fermented (48 h) with 1% (mass to volume ratio) of different microbial cultures, respectively. Effects of different fermentations on MF and ML were determined based on the changes of total phenolics (TPC), flavonoids (TFC), anthocyanins, total sugar, DPPH activity, ferric reducing antioxidant power (FRAP), albumin denaturation inhibition activity (ADI), anti-lipoxygenase activity and α-amylase inhibition activity (AI). Results: Generally, ML had higher AI than MF. However, MF exhibited higher DPPH, FRAP and anti-lipoxygenase activity than ML. After all forms of fermentation, DPPH and AI activity of MF and ML were increased significantly (P < 0.05). However, the effects of fermentation on TPC, FRAP, ADI and anti-lipoxygenase activity of MF were in contrast with ML. TPC, FRAP and anti-lipoxygenase activity of ML were enhanced, but reduced in MF after fermentation. Although the effects exerted by different microorganisms in MF and ML fermentation were different, the bioactivities of MF and ML were generally improved after fermentation. Fermentation by Tempeh starter enhanced TPC (by 2-fold), FRAP (by 2.3-fold), AI (at 10% increment) and anti-lipoxygenase activity (by 5-fold) of ML, whereas Tapai fermentation effectively enhanced the DPPH (at 17% increment) and ADI (by 2-fold) activity of MF. Conclusion: Findings of this study provide an insight into the future process design of MF and ML processing into novel functional foods.

ABSTRACT

Scutellariae Radix(Huangqin) is a well-known traditional Chinese medicine(TCM) used for the treatment of clearing heat in clinical application. It is bitter-cold by using directly, but the bitter-cold property can be relieved after wine-frying. The study of taste changes before and after wine-frying of Scutellariae Radix is of great significance in identifying Scutellariae Radix and wine-processed Scutellariae Radix and clarifying the traditional theory of wine-processing. In this experiment, 10 batches of Scutellariae Radix and wine-processed Scutellariae Radix were prepared. The contents of 5 flavonoids were determined by high performance liquid chromatography(HPLC), and principal component analysis(PCA) was performed with 5 flavonoids as variables. As a result, the contents were different in different batches of Scutellariae Radix, but Scutellariae Radix and wine-processed Scutellariae Radix could not be distinguished. Five sensory attributes(sour, salty, fresh, sweet, and bitter) were evaluated by artificial tasting, and the response values of 7 sensors(AHS, AHS, PKS, CTS, NMS, CPS, ANS, SCS) representing the taste of pieces were detected by electronic tongue. The correlation between sensory evaluation and response values of the electronic tongue were analyzed, and the results showed that the sensory evaluation of sour, salty, fresh, sweet, bitter and AHS, CTS, NMS, ANS, SCS sensors had different degrees of correlation, indicating that the electronic tongue technology can be used as an alternative to artificial taste and can serve as a means for quantifying the taste, and it can be used to evaluate the taste of TCM pieces. The taste method was used to analyze the response values of the electronic tongue, and the results showed that the bitterness of wine-processed Scutellariae Radix decreased and the salty taste increased. PCA was used to analyze taste changes before and after wine-processed Scutellariae Radix, and the results showed that taste differences between 2 pieces were divided into 2 categories. PCA loading scattering plots showed that response of saltiness and bitterness were the major factors to affect overall taste in Scutellariae Radix and wine-processed Scutellariae Radix. Based on electronic tongue response values, the Fisher discriminant model for Scutellariae Radix and wine-processed Scutellariae Radix was established, which showed that it could effectively discriminate them with a recognition rate of 100%. The experimental results showed that the electronic tongue combined with multivariate statistical analysis can be used to evaluate taste of TCM, at the same time, it could provide a fast and simple method for identifying different processed products.

ABSTRACT

Objective:To observe the changes of serum adiponectin and uric acid levels in patients with type 2 diabetes mellitus (T2DM) , and to investigate its correlation with diabetic retinopathy (DR) .Methods:Data of 120 hospitalized T2DM patients in our hospital from Jun. 2017 to Jun. 2019 were retrospectively analyzed. According to the fundus findings, they were divided into no DR group (40 cases) , hyperplastic DR group (45 cases) and non-proliferative DR group (35 cases) . 30 healthy cases in the same period were selected as the control group. The level of aboratory biochemical indexes, serum adiponectin and uric acid were compared between the 4 groups. Risk factors affecting DR were analyzed by Logistic multivariate regression.Results:There was no significant difference in TC among the four groups ( P>0.05) . There were significant differences in TG, LDL-C, HDL-C, HbA1c, and FPG among the four groups ( F=1.638, 0.582, 2.715, 5.382, 5.174, P=0.007, 0.016, 0.028, 0.003, 0.001) . TG, HbA1c, and FPG of DR group, non-proliferative DR group and proliferative DR group were significantly higher than those of the control group (no DR group vs the control group: t=2.246, 12.440, 14.539, P=0.028, 0.000, 0.000; Non-hyperplastic DR group vs the control group: t=3.127, 14.775, 14.767, P=0.000; Hyperplastic DR group vs the control group: t=3.349,16.197,12.755, P=0.001,0.000,0.000) , and the level of these indexes were rising with the aggravation of the disease. HDL-C of DR group, non-proliferative DR group and proliferative DR group were significantly lower than that of the control group (non-hyperplastic DR group vs the control group: t=2.113, P=0.040; DR group vs the control group: t=2.778, P=0.007) , and decreased with the aggravation of the disease. There were significant differences in the level of serum adiponectin and uric acid among the four groups ( F=4.728, 26.491, P=0.004, 0.001) . The level of serum adiponectin was significantly lower in no DR group, non-proliferative DR group and proliferative DR group than that in the control group (no DR group vs the control group: t=2.227, P=0.031; Non-hyperplastic DR group vs the control group: t=5.198, P=0.000; Hyperplastic DR group vs the control group: t=6.827, P=0.000) , and decreased with the aggravation of the disease. The level of serum uric acid in the DR group, non-proliferative DR group and proliferative DR group were significantly higher than those in the control group (No DR group vs the control group: t=5.681, P=0.000; Non-hyperplastic DR group vs the control group: t=13.688, P=0.000; Hyperplastic DR group vs the control group: t=23.620, P=0.000) , and increased with the aggravation of the disease. Multivariate Logistic regression analysis showed that the course of diabetes, HbA1c, adiponectin and uric acid were independent risk factors affecting DR. Conclusions:The level of serum adiponectin decreases in DR patients, and the level of serum uric acid increases in DR patients, which relates to DR progress. Adiponectin and uric acid are risk factors affecting DR.

ObjectiveTo investigate the mechanism of action of miR-196b in regulating the growth and apoptosis of hepatoma cells by targeting nuclear apoptosis-inducing factor 1 (NAIF1). MethodsReal-time PCR was used to measure the expression of miR-196b in hepatoma HuH-7, SNU-449, HepG2, and SMCC7721 cells versus normal human HL7702 hepatocytes. The hepatoma HepG2 cells were collected and divided into Control group (blank control), Anti-NC group (transfected with inhibitor control), Anti-miR-196b group (transfected with miR-196b inhibitor), si-NC group (transfected with siRNA control), si-NAIF1 group (transfected with NAIF1 siRNA), Anti-miR-196b+si-NAIF1 group (co-transfected with miR-196b inhibitor and NAIF1 siRNA), and Anti-miR-196b+si-NC group (co-transfected with miR-196b inhibitor and siRNA control). MTT assay was used to measure the change in proliferation, plate colony formation assay was used to measure colony formation ability, flow cytometry was used to measure cell apoptosis, and Western blot was used to measure the protein expression of Bax and C-caspase-3. Target gene prediction software predicted that NAIF1 might be a target gene of miR-196b, and the luciferase reporting system was used to identify the targeting relationship. The t-test was used for comparison of continuous data between two groups; a one-way analysis of variance was used for comparison between multiple groups, and the SNK-q test was used for further comparison between two groups. ResultsThere was a significant difference in the expression level of miR-196b between hepatoma HuH-7, SNU-449, HepG2, and SMCC7721 cells and normal human HL7702 hepatocytes (1.85±0.16/1.63±012/2.36±0.25/1.92±0.13 vs 1.00±0.09, F=29.05, P<0.001). Compared with the Anti-NC group, the Anti-miR-196b group had significant reductions in the expression level of miR-196b (0.42±0.03 vs 1.02±0.10, P<0.05), cell proliferation (0.20±0.02 vs 0.30±0.05, P<0.05), and colony formation ability (64.35±6.97 vs 119.54±11.82, P<0.05) and significant increases in apoptosis rate (22.30%±2.09% vs 4.26%±0.35%, P<0.05) and relative protein expression of Bax (0.69±0.08 vs 0.30±0.05, P<0.05) and C-caspase-3 (0.63±0.05 vs 0.21±0.04, P<0.05). Compared with the si-NC group, the si-NAIF1 group had significant increases in proliferation ability (0.46±0.05 vs 0.31±0.04, P<0.05) and colony formation ability (138.92±9.66 vs 118.47±838, P<0.05) and significant reductions in apoptosis rate (4.12%±0.40% vs 1.23%±0.12%, P<0.05), NAIF1 (0.10±0.01 vs 0.17±0.02, P<0.05), and protein expression of Bax (0.18±0.02 vs 0.29±0.03, P<0.05) and C-caspase-3 (0.12±0.01 vs 020±0.03, P<0.05). Compared with the Anti-miR-196b+si-NC group, the Anti-miR-196b+si-NAIF1 group had significant increases in proliferation ability (0.28±0.02 vs 0.21±0.03, P<0.05) and colony formation ability (97.12±8.23 vs 66.35±5.20, P<0.05) and significant reductions in apoptosis rate (9.60%±1.11% vs 21.14%±1.32%, P<0.05), NAIF1 (0.30±0.04 vs 0.52±0.06, P<0.05), and protein expression of Bax (0.28±0.03 vs 0.67±0.06, P<0.05) and C-caspase-3 (0.22±0.05 vs 0.60±004, P<0.05). ConclusionDownregulation of miR-196b can inhibit the growth and induce the apoptosis of hepatoma cells via negative regulation of NAIF1.

ABSTRACT

To establish a method for the determination of polymer impurities in cefixime raw materials and preparations, a cefixime degradation solution containing polymer impurities was prepared by forced polymerization. Polymer impurities in the degradation solution were separated and identified by high performance gel chromatography and the column switching-LC-MSn method. A new RP-HPLC method for cefixime polymer was established and validated with a Phenomenex Gemini-C18 column using a mobile phase gradient elution of 0.5% formic acid-water solution and 0.5% formic acid-acetonitrile solution. The results showed that when using this high performance gel chromatography method some small molecular weight impurities were co-eluted with the polymers, resulting in a poor specificity and poor quantitative accuracy. But when using the RP-HPLC method, three polymer impurities were detected with good specificity, sensitivity and robustness, including two cefixime dimers, and dehydrate dimer. Therefore, the described RP-HPLC method is suitable for the quality control of polymer impurities in cefixime, and cefixime degradation solution can be used as suitable solution for analysis of cefixime polymers.

@#【Objective】 To investigate the efficacy and safety of ultrasound- guided above- knee and below- knee radiofrequency ablation for the treatment of saphenous varicose veins.【Methods】Patients who underwent ultrasound-guided radiofrequency ablation closure in our department from July 2019 to November 2019 were compared in operation time , recovery time,volume of sclerosant foam,pain score,venous clinical severity score(VCSS),Aberdeen Varicose Veins Questionnaire (AVVQ), and complications. 【Results】 Fifty- nine patients underwent above- knee radiofrequency ablation and 19 patients underwent below-knee radiofrequency ablation. The average operation time(69.75 vs. 78.95)min, time return to normal activity(2.93 vs. 3.58)min or the volume of foam(28.3 vs. 24.2)mL were similar in both groups. The pain score,VCSS,and AVVQ scores 24 h,1 week,or 4 w postoperative decreased significantly in the two groups. No deep vein thrombosis,pulmonary embolism,or infection occurred in the two groups after surgery. Other complications including phlebitis,pigmentation,bleeding,rash,or paresthesia,showed no difference in rates. And overall incidence of complications were similar between the two groups. 【Conclusions】 Both above-knee and below-knee radiofrequency ablation are safe and effective treatments for great saphenous varicose veins.

ABSTRACT

OBJECTIVE@#To observe the effect of lumbar disc herniation treated with acupuncture at different time intervals.@*METHODS@#A total of 180 patients of lumbar disc herniation were randomized into an observation group 1, an observation group 2 and an observation group 3, 60 cases in each one. All patients were treated with acupuncture at Jiaji L-L (EX-B 2), Huantiao (GB 30), Weizhong (BL 40), etc. And then KWD-808 electroacupuncture instrument was connected, time intervals of acupuncture were once every day, once every 2 days and once every 3 days, 3 weeks were provided. At 1-week, 2-week and 3-week treatment, the visual analogous scale (VAS) and Japanese Orthopedics Association (JOA) scale were observated, and the effects were evaluated.@*RESULTS@#The effective rates in the observation group 1 and the observation group 2 were 96.7% (58/60) and 95.0% (57/60), there was no significant different between the two groups (>0.05), which were superior to 88.3% (53/60) in the observation group 3 (both 0.05) at 1-week, 2-week and 3-week treatment, however, they were significantly reduced compared with the observation group 3 (all 0.05) at 1-week, 2-week and 3-week treatment, however, they were significantly increased compared with the observation group 3 at 3-week treatment (both <0.05).@*CONCLUSION@#Acupuncture once every day and once every 2 days in the treatment of lumbar disc herniation is equally effective, better than once every 3 days.

Objective@#To investigate the current status of professional identity among the undergraduates majoring in public affairs management and the related influence factors.@*Methods@#A stratified cluster sampling method was used to conduct a questionnaire survey among the 115 undergraduates majoring in public affairs management in a medical university in Harbin, China, and the contents of the questionnaires included general status and professional identity. The t-test, one-way analysis of variance, and multiple linear regression analysis were used to analyze the influencing factors of professional identity. In-depth interviews were conducted for some students to explore the profound influencing factors for professional identity among the students majoring in public affairs management.@*Results@#The overall level of professional identity was low among the undergraduates majoring in public affairs management in the medical university in Harbin, and the score of professional identity was 3.25±0.56. The scores of cognitive dimension, emotional dimension, behavioral dimension, and applicable dimension were 3.41±0.69, 3.21±0.78, 3.24±0.71, and 3.13±0.72, respectively. The multivariate analysis showed that grade, academic performance, major selection, and children number in family had significant influence on their professional identity. The students who were at grade four, who had high academic record (b=-0.276, P<0.01), who chose this major independently (b=-0.277, P<0.01), and who were the only child (b=-0.233, P<0.05) had a high score of professional identity. As for the undergraduates at different grades, the undergraduates at grades two and three had a significantly lower score of professional identity than those at grade four (b=-0.296 and -0.369, P<0.05), while there was no significant difference between the undergraduates at grades one and four (b=-0.008, P>0.05). Furthermore, the students with a high score of professional identity had higher possibility to pursue the job related to this major than those with a low score (3.39±0.53 vs. 2.92±0.49, t=4.414, P<0.01).@*Conclusion@#Universities should strengthen the publicity of the major of public affairs management, optimize the curriculum, and add job experience, so as to enhance professional identity among students with this major and provide talent for the field of public health management.
